Is this a horizontal root fracture at the apex? (Short RCT case)

I had my first mono-rooted root canal case (tooth #11) about week ago (due to the pandemic, I was only able to work on endodontic models). Unfortunately, it did not go as planned. In our clinic, we still use hand files and we don’t have an apex locator. https://i.imgur.com/1ckIuI9.png (pre-op) I encountered my first problem during shaping of the canal. I measured the root via xray - 25mm. But most of our files were exactly 25mm without the rubber stopper. I took an x-ray using the #15 k-file at 25mm. It seemed to stop exactly at the apex. https://i.imgur.com/uekDKpC.png But the patient would ..read more

I am currently working as an associate at a dental practice where the schedule is almost non existent. There are days when there are no cases for weeks. The owner today told me that I need to figure out a way to produce more and apparently be nice to patients. I am probably the nicest dentist in my area who genuinely cares about patients, takes time to emphasise preventive care none of which the main dentist does. Somehow the revenue generation is suddenly a problem after 6 months. While I do agree that I need to earn my keep, if the patients don’t really care or follow up what can I do as an ..read more

Yes yes I did learn removable in school, im just curious to know how others work, i have friends for example who do custom trays and wax rims for every case even if possible to hand articulate Do you design your rpds? Do you always use custom trays? If not, do you only use alginate? Do you modify your universal stock tray with wax ? Do you use alginate adhesive? When do you use wax rims? I just want to simplify my workflow, dont have a scanner yet submitted by /u/markthelegacy [visit reddit] [comments ..read more

Roundhouse bridges

So I have this new patient that comes in, primary complaint is her #6-#8 PFM bridge is getting old, shade doesn't look great, and wants it replaced. Turns out it's slightly mobile and that all of her anteriors and premolars are mobile to some extent due to extremely short roots, possibly ortho related. Overall she has good hygiene , is fairly lightly restored, and has solid bone on her molars. We get to talking more about options and she says she doesn't like the idea of implants due to pain/healing time and is "too young" for removable. She has high aesthetic demands and would prefer that al ..read more

The isolation is a disaster, multiple teeth isolation in such cases is so difficult. Yesterday I was doing a deep margin on a mandibular second molar and couldn't get it well isolated so I referred it. What do you do in such cases? submitted by /u/khanzir_basha [visit reddit] [comments ..read more

This is quoted in AAE management of endodontic emergencies - 'Moreover, more than 50% of endodontists preferred complete instrumentation compared to pulpectomy-only procedures, especially in necrotic cases. Patient presents with dull ache upper left side and thinks it the last tooth (had deep decay restored in 2021). Dull ache started a week ago and consistent throughout the day. Percussion wnl for all upper and lower left side teeth. Cold testing: all normal, except for upper second molar which was testing negative for cold (no response) Testing done multiple times to make sure wasn't a false reading Open the tooth for RCT and pulp tissue noted. For teeth that are necrotic I expect there to be no pulp or bleeding from the tooth. Was my testing / diagnosis wrong or can a necrotic tooth still pre ..read more

Data Question

Hello all! I am a healthcare consultant and have recently started working with more dental practices. I am looking to see if anyone has any good recommendations of a dental database? We are looking to analyze business practices in dental offices and was looking for some benchmarks and industry standards that practices use. I have gone through the data provided from ADA but did not see much so was wondering if anyone had any information or data analytics on business practices in dental offices. Insurance billing reports, patient count, code frequency use, etc. TYIA submitted by /u/Odd_Scarcit ..read more
