Nearly one-quarter (23%) of the RLNs deviated from their anticipated anatomical course, 30% of RLNs were fixed, splayed, or entrapped at the level of the capsule of the thyroid. At ten years there was no difference what surgery they had. Previous article by Tufano in the same Thyroid Journal found thyroid radiofrequency RFA was as good as surgery for micropapillary cancers, with less pain, no scar, loss of work, less expensive, and social and emotional outcomes were better. Dr.Guttler’s comments: Another great study by Dr.Baek’s group on the use of ethanol for TGD cysts. 81 patients with with TGD cysts had PEI. 68/81 were seen in follow up. 55/68 81 % success rate from first treatment. 1/68 has a wound inflammation. 42/68 were followed for 24-131 months ( mean 69 months). Long term success rate was 83%. Size decease and symptom and cosmetic issues resolved. No cancers in the ablated TGD cyst group developed cancer. Thyroid May 2018 Lopresti J.L. et al DR.Guttler’s comments: Article from USC thyroid Lab on orders of over 400,000 thyroid tests. 400,000 orders for thyroid tests. 90,000 orders for rT3. Benign thyroid nodules class 11 are good candidates for Ablation. However, Class III will need more studies to be sure you don’t have a nodule with potential for thyroid cancer. A suspicious needle biopsy alone can lead to unnecessary thyroid surgery for a benign nodule. A suspicious molecular marker result for your class III cytology is still not a good way to avoid surgery for a benign nodule.
